Birth Certificate, Commercial Driver License (CDL), Social Security Card; and if applicable, Resident Alien Card, permanent Resident Card.

List of dependents' names, Social Security numbers and birth dates for who you will be enrolling insurance benefits.

If planning on DIRECT DEPOSIT for your check, bring a voided check with bank routing number and account number. A direct deposit form from your bank with this information is also acceptable.

If participating in the Rider Program, your Rider must bring photo ID and birth certificate (passport / government visa is also acceptable).

Clothes to last 7-10 days. We will make every attempt to get you by the house within 7days of dispatch. (DO NOT pack any of the following: Tank tops /sleeveless shirts, and shirts with lewd, offensive, or suggestive matter.)

Medications. DO NOT stop taking them or leave them at home.

Towels, washcloths, and all other necessary personal grooming items (shampoo, soap, toothpaste, deodorant, etc.)

- Lodging and meals (breakfast, lunch, dinner while participating in orientation) at Celadon's corporate campus in Indianapolis are provided to you at no cost.
