This is another reminder to complete the smokelyzer and tobacco use affidavit in order to have the non-tobacco user discount applied to your medical premium. These must be completed and turned in before 11:59 p.m. on December 23, 2015. If you do not meet this deadline, you will pay the standard insurance premium amount, beginning January 1, 2016.
To complete the smokelyzer test, you will need to go to the Verve Clinic at headquarters in Indianapolis. It will only take a few minutes of your time, and you DO NOT NEED AN APPOINTMENT.

If your location is not listed, please contact Benefits at 844-354-5896 or firstname.lastname@example.org. All tobacco affidavits MUST be returned to the benefits department by the December 23rd deadline even if your location's smokelyzer test is after that deadline!
YOU MUST COMPLETE AND TURN IN BOTH THE TEST RESULTS AND THE AFFIDAVIT IN ORDER TO RECEIVE THE DISCOUNT.
The tobacco affidavit is a legal document that you sign stating that you (and your spouse if applicable) do not use tobacco products and have not for the past 12 months.
Both of these documents can be emailed to email@example.com, mailed to the Benefits department, or faxed to 317-829-6366.
Also, it is imperative that you complete a spousal affidavit if you wish to have your legally married spouse on your health insurance. To qualify, your spouse must be unemployed or not be offered health insurance through their employer. This document must also be turned in by the December 23rd deadline.
